Visit our member benefits information webpage to learn more about your benefits through Advantage Dental.
How Do I Choose a Dentist?
Advantage Dental assigns each new member to a Primary Care Dentist (PCD) in their region that is the closest to them and accepting new patients. If you want to change your PCD, you can call Member Services at 1-866-268-9631. You can also view the Provider Directory for a full list of Advantage Dental providers.
Why Did I Receive a "Confirm a Dental Service" Form?
Advantage Dental needs to confirm that we only paid for the dental services you received. You can help us by completing and returning this form if you get one.
Why Did I Receive a Member Survey?
Advantage Dental sends surveys to members after they receive dental services. These surveys help us know what went well and where we can improve. If you receive a survey, please complete it and mail it back in the pre-paid envelope provided.
Why Did I Receive a “Notice of Action Benefit Denial” Letter?
This letter was sent to you to let you know that a dental service was denied. If you think the service should be covered, follow the instructions in the letter. Call Member Services at 1-866-268-9631 for help.
Can I Get a Second Opinion?
A second opinion is available at no cost to you. Call Member Services at 1-866-268-9631 for more information.
